About The Position

Sadler Health Center is seeking Community Health Workers to join their team, with positions available at their Carlisle or West Shore Clinic. The role involves establishing and maintaining trusting relationships with patients and their families to assess their needs and identify appropriate programs. Community Health Workers will collaborate closely with a team of care coordinators, patient enrollment specialists, pharmaceutical support staff, and case managers. Key responsibilities include supporting patients and families in establishing primary care for various health needs (medical, dental, vision, behavioral health), assisting them in navigating systems to obtain resources, and arranging transportation for appointments. The position also requires facilitating communication between providers, patients, and loved ones to ensure service needs are met. Communication will involve both phone interactions and face-to-face visits. Travel within Perry and Cumberland counties is a requirement. Sadler Health Center operates a unique, team-oriented care model providing comprehensive primary care, including medical, dental, behavioral health, medication-assisted treatments, and case management services. Requirements

  • Works independently and is self-directed
  • High School Diploma or GED
  • Organizes, prioritizes, and coordinates multiple activities and tasks
  • Graduate from an accredited Medical/ Dental assisting training program
  • Proficiency in the use of Microsoft Office applications; Word, Excel and Outlook
  • Customer Service experience
  • Effectively communicates orally and in writing
  • Ability to effectively and professionally represent the organization
  • Ability to work in a team environment
  • Remains calm and effective in high pressure and emergency situations
  • Ability to organize data and medical records in a manner consistent with national standards
  • Demonstrates knowledge of medical terminology
  • Must be dependable and able to maintain punctual attendance and willingness to accommodate a flexible work schedule

Nice To Haves

  • Bilingual language skills
  • Secondary education in a clinical area of focus such as Certification as a Medical Assistant or Community Health Worker
  • Experience working with a diverse patient population which may include low income, multi-ethnic populations in a community health clinic setting
  • Electronic Health Record experience
  • Experience in case management
  • Experience with data reporting/healthcare information technology

Responsibilities

  • Establish, build and maintain trusting working relationships with patients and their families
  • Assess and identify appropriate programs patients may need or are eligible for
  • Work closely with care coordinators, patient enrollment specialists, pharmaceutical support team and case managers
  • Provide support to patients and their families to establish primary care for health, dental, vision, behavioral health
  • Assist patients and navigate systems to obtain resources or arrange for transportation to appointments when needed
  • Assist in communication between providers and patients and involved loved ones to ensure service needs are achieved
  • Maintain communications by both phone interaction and face to face visits
  • Travel within Perry and Cumberland county
  • Acquire Community Health Worker Certification within 12 months
