About The Position

Sentara Community Care Center Opitz is hiring a Full-Time Community Healthcare Worker in the Woodbridge, VA area. This role involves working in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care. The Community Healthcare Worker will engage in interventions to improve self-management skills and enhance the quality of life for patients dealing with chronic and/or complex illnesses. Key aspects of the role include providing education, implementing behavior modification strategies, and offering social support to patients and their families. The individual will also act as a patient advocate and healthcare navigator, assisting with resource allocation and management.

Requirements

  • Virginia Driver's License - Active/current and in good standing
  • High School or equivalent

Nice To Haves

  • Bilingual in English and Spanish
  • Experience working in a community-based setting
  • Knowledge of medical terminology
  • Insurance knowledge/experience
  • Previous experience in outreach/community service
