About The Position

A Community Health Worker (CHW) serves as a vital link between patients' health and social services. As trusted members of the community, CHWs bridge gaps in care by facilitating access to essential services, providing health education to support the management of chronic conditions, and offering individual support in advocating and navigating health services. Through their role, CHWs support community members in improving health outcomes and building strong relationships that encourage positive lifestyle changes. Community Health Workers are frontline public health workers. They are trusted members of the community who have a close understanding of the communities they serve. A Community Health Worker builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities, which include outreach, individual and community education, social support, and advocacy.

Requirements

  • Computer skills include web browsing, Excel, and EMR software
  • Patient and community education background
  • Excellent people skills; ability to work independently and as a team member
  • Excellent organizational skills
  • Understanding of HIPAA regulations, respect for patient confidentiality
  • Efficiency and attention to detail
  • Knowledge of chronic health conditions that are prevalent in Native American community.
  • Must have a valid certificate from the Minnesota State Colleges and Universities (MnSCU) demonstrating completion of an approved community health worker certification program.
  • Two years of direct care experience working with mental health and substance use disorders

Responsibilities

  • Provide individual and group education on mental health and substance use and related public health concerns to NACC clients and the Native American community
  • Provide information and referral to NACC and other partnering programs to meet the comprehensive needs of clients
  • Identify, create, and implement client education materials to share in groups and at community events
  • Work with clinical staff to develop program protocol and workflow implementation
  • Explore patient resources that support success, create resource tools to be used in the clinic and community
  • Respond to and fulfil requests for community public health education
  • Provide harm reduction client education and educational materials
  • Collect and maintain up-to-date data related to grant outcomes and report regularly
  • Active member in supporting harm reduction activities and services
  • Support clients with health-related goals and objectives outlined in their chemical mental health treatment plans.
  • Support clients in tracking their progress
