About The Position

The Youth Tele Behavioral Health Program provides students with year-round long term or short term counseling via tele and community resources. The Community Health Worker works alongside a Behavioral Health Clinician and school personnel to help students and families access services and resources. Work closely with Clinician to organize school based behavioral health program and care coordination. The Community Health Worker provides case management; ensuring students and families receive the help they need. Document encounters; collect data; complete monthly reports; maintains comprehensive electronic client files, which include referrals, release of information, consents and other medical documentation.

Requirements

  • Must have a HS Diploma or GED.
  • Experience in Human Services or Social Services (paid, volunteer, life experience)
  • Strong computer skills- including Google suite & Zoom
  • Knowledge of area community agencies and resources
  • Skill in organizing resources and establishing priorities
  • Creative and analytical thinking
  • Strong interpersonal and communication skills

  • Completed all Heywood Healthcare and Department Specific Mandatory requirements in the prior calendar year.
  • Reports Incidents in a timely and effective manner.
  • All employees with direct patient contact are responsible for actively engaging in the practice of, monitoring and enforcing compliance to hand hygiene.
  • Provide home, school and community based care coordination to youth and their families.
  • Assist students and families to access on site tele behavioral counseling services as well as off-site community based clinicians and non-clinical resources (as needed)
  • Conduct comprehensive screening/assessment activities including mental health status, health screen and social needs factors
  • Coordinate, manage and schedule tele behavioral counseling sessions
  • Assist with obtaining insurance authorizations for clinical sessions as needed
  • Provide ongoing follow-up, motivational interviewing and goal setting with students/families
  • Continuously expand knowledge and understanding of community resources and services and implement personalized case management for student and family needs
  • Collaborate as a member of the school’s guidance team to coordinate care for students in need
  • Participate in regular supervision and program staff meetings
  • Track and submit data; Complete monthly reports
  • Manage time and resources in a skilled manner
  • Understand school culture and participate in school events
  • Demonstrate appropriate boundaries in all interactions
  • Attend relevant trainings, conferences, community events
  • Follow Heywood’s policies and procedures; Maintain confidential information
  • Additional duties will be at the discretion of your supervision
