Community Health Worker
About The Position
Join our team as a Community Health Worker I (CHW I), where you'll play a vital role in enhancing patient care by addressing social determinants of health. Reporting to the Manager of Care Management, you will provide bedside and community support to patients, ensuring they receive comprehensive care from admission through post-discharge. Your responsibilities will include conducting SDOH screenings, enrolling patients in community programs, delivering patient education, coordinating appointments, and engaging with patients to reinforce treatment plans. As a key collaborator with physicians, nurses, care managers, and community partners, you will actively participate in community engagement activities and assist with volunteer coordination. Reliable transportation is essential for home visits and community outreach.
Requirements
- High school diploma or equivalent; CHW Certification required.
- Proficient in spoken and written English.
- Familiarity with social/health issues and medical terminology is a plus.
- Required training must be completed within 30 days of hire.
- Strong interpersonal communication, critical thinking, and the ability to identify needs for professional intervention in patient care.
- Reliable transportation is essential for home visits and community outreach.
