About The Position

The FIT Program Community Health Worker will provide outreach, counseling, navigational, and case management services to patients recently released from prison and jail into Wake County with a diagnosis of a serious mental illness (for example, schizophrenia, schizoaffective disorder, or bipolar disorder) in addition to chronic medical conditions and/or substance use disorders who are referred by prison/jail health providers prior to prison release. Patients will receive psychiatric and primary care services at the UNC REACH (Recovery and Access to Comprehensive Health) Enhanced Primary Care clinic. In order to provide outreach services in the community, Community Health Workers must possess a valid driver’s license and the ability to operate a vehicle safely and responsibly to perform essential job functions, including community outreach and client support. Work Schedule Monday – Friday, 8am – 5pm
