About The Position

The Community Health Worker – Housing Stabilization & Navigation provides direct housing stabilization services to individuals and families at risk of homelessness. This role carries an active caseload and is responsible for comprehensive prevention assessments, individualized housing stabilization planning, landlord mediation, financial assistance coordination, and cross-program navigation support. This role requires consistent, timely documentation in both HMIS and Salesforce Nonprofit Cloud (NPC) as part of coordinated care delivery and funding compliance. This role is non-supervisory.

Requirements

  • Bachelor’s degree in social work, public health, or related field preferred (or equivalent experience).
  • Minimum two (2) years of experience in housing case management or eviction prevention.
  • HMIS experience required.
  • Experience with landlord mediation preferred.
  • Experience working with diverse and vulnerable populations preferred.
  • Strong problem-solving and mediation skills.
  • Ability to manage complex housing cases.
  • Strong organizational and documentation skills.
  • Cultural humility and trauma-informed practice.
  • Strong written and verbal communication skills.
  • Ability to maintain strict confidentiality.
  • Valid CA Class C Driver’s License and proof of auto insurance.
  • Reliable transportation for local travel.
  • Ability to work occasional extended hours based on program needs.
  • Employment is contingent upon completion of a criminal background check. Findings will be reviewed in relation to job responsibilities and in accordance with applicable laws.
  • Must comply with any immunization, health screening, or safety requirements required by partner sites or public health regulations.

Responsibilities

  • Conduct comprehensive housing prevention assessments.
  • Develop individualized housing stabilization plans.
  • Carry and manage an active caseload within established limits.
  • Provide ongoing tenancy support and crisis planning.
  • Facilitate communication between tenants and property owners.
  • Negotiate payment arrangements and address lease compliance issues.
  • Support eviction prevention strategies.
  • Assist clients in accessing rental and utility assistance.
  • Ensure complete documentation per PSAP standards.
  • Upload required documentation into HMIS.
  • Screen for eligibility for additional services (e.g., CalAIM).
  • Coordinate referrals across internal Program Areas.
  • Participate in case conferencing and interdisciplinary collaboration.
  • Complete timely and accurate documentation in all required data systems, including HMIS and Salesforce Nonprofit Cloud (NPC), within established program timeframes.
  • Ensure all client assessments, housing stabilization plans, landlord mediation activities, and financial assistance requests are recorded consistently across systems.
  • Maintain compliance with Sacramento CoC data standards, PSAP requirements, and internal organizational data policies.
  • Support data quality and validation processes to ensure accuracy, completeness, and reporting readiness.
