About The Position
Come join the community outreach team at Mercy Medical Center, part of Trinity Health Of New England!! Position Purpose This is a grant funded position. The Talent Acquisition Partner will be able to provide more details in the form of a Grant Funded Position Agreement. In this grant the Community Health Worker assists homeless individuals in the local community gain access to key social service and healthcare providers/services to better their health and improve their lives.
Requirements
- CHW Level I- Minimum high school diploma or equivalent
- CHW Leve II- Minimum Bachelor's Degree in human services or related field
- Basic Life Support (BLS) certification
- Current Driver's License in good standing

- BILINGUAL (SPANISH/ENGLISH) STRONGLY PREFERRED
Responsibilities
- Improve access to health and human services for homeless population through in-person interactions, telephone contacts, and coordination of referrals
- Facilitate or conduct enrollment in available federal, state and local programs to increase access to health care and support services
- Maintain frequent contact with community agencies serving vulnerable homeless and at-risk of homelessness populations
- Provide individual & community support. Serves as a community liaison between target population and agencies and providers
- Provide transportation assistance necessary to ensure the success of the populations served
