WHAT YOU CAN EXPECT Schedule medical appointments and transportation, conducting reminder/confirmation phone calls, and completing referrals to any additional services. Collect vitals; disease management, including symptom tracking and reporting, health education/prevention and maintenance of patient's supplies and durable medical goods. Coordinate with health care providers to report on patient progress and conference, if intervention needs to be modified or discontinued. Work with community agencies, social workers, health educators, peer navigators, and health care providers to link patients to needed preventative health services, to prevent illness, improve skills for self-management of chronic conditions, and promote healthy lifestyle behavior change. Accompany patient to appointment as needed; may be virtually; assist patient navigating the medical system. Enter and maintain electronic records, compile report and complete other program documentation in a timely manner. Play a consistent and active role in identifying project inefficiencies and finding collaborative solutions to the problems.

Requirements

  • Required High School Diploma/GED

Nice To Haves

  • 1 year Medical Assisting in an outpatient setting Preferred
  • Community Health Worker Certification (CHW) - Illinois - Community Health Worker Advisory Board Indiana- Affiliated Service Providers of Indiana ASPIN certification-preferred

Responsibilities
