The Community Health Worker (CHW) Coordinator is experienced in the work of a CHW to provide support, education, and advocacy to individual clients and to the community. The CHW Coordinator should be someone knowledgeable of the community culture and languages spoken in the geography of service, showing a high level of cultural competence. The CHW Coordinator demonstrates strong leadership and ensures the CHW team provides clients and community members with accurate information about health and community services with the goal of reducing identified health disparities. The CHW Coordinator also monitors and drives toward the achievement of all program goals and objectives providing consistent oversight and support to CHW team. The Community Health Worker Coordinator creates and monitors all CHW assignments as appropriate, which may include client caseload, program documentation, as well as the daily human resources activities relative to CHW work schedules, attendance, and performance.
