This position will lead and coordinate daily operations in a variety of patient care activities. The individual will assist in the delivery of health care and patient care management in accordance with scope and licensure and will take an active lead in facilitating identified performance improvement activities within the department. Requirements

  • High School diploma or GED equivalency preferred.
  • Experience in a health care setting and/or in an office preferred.
  • Previous experience as a Community Health Worker, or similarly related role, preferred.
  • A builder of patient autonomy and problem-solving capacity. A strong advocate for patients when the solution to a patient's problem lies beyond their capacity to obtain it on their own.
  • An exceptional communicator, which the CHW manifests through internal communications, accurate, consistent, and timely documentation, as well as other forms of communication as needed for specific niche.
  • Able to multi-task.
  • Can work under pressure and meet deadlines.
  • Able to work with a high degree of precision and attention to detail.
  • Able to react calmly and effectively in emergency situations.
  • Great telephone and computer skills.
  • Able to work unsupervised.
  • Performs other required duties as assigned in accordance with scope of role.

Responsibilities

  • Improves access and equity to internal (ODHS) and external (community) resources.
  • Is able to delegate or refer patients to other experts, both internal and external, when patient's issues fall beyond the scope of the CHW's role or expertise.
  • Gain and maintain knowledge of related social services programs.
  • Trusted member of, or deeply understands, the community he/she serves
  • Internal referral liaison between health and social services and the community
  • Keep working relationships and act as a liaison for patients with medical staff, behavioral health staff, internal staff, other agencies.
  • Maintain ODHS standards, policies, procedures, and regulations, in accordance with local, state, and federal laws and with ethical standards of practice.
  • Scaffold patients as they learn skills and utilize resources until they are able to do so independently.
  • Maintains high level of confidentiality while interacting with patients and clients with empathy and respect
  • Prepare and maintain written case records, reports, and forms, including case follow up and closing.
  • Travel to other locations and attend outreach events with applicable.
  • Consistent and knowledgeable execution of niche or department-level programming
  • Leverages niche skills, certifications, and experience to benefit patients who would benefit from that expertise
  • A strong ability to collaborate and maintain working relationships with co-workers, relevant internal departments as well as external community agencies.
  • Interview, intervene, and resolve for patient circumstances regarding financial, social, emotional, health, and other problems.
  • Research and resolve patient or client issues, including consulting with providers, peers, and management
  • Assist with special projects and other duties as necessary and required
  • Attending internal and external meetings as needed and required.
  • Good communication skills and ability to work closely with others and in teams.
  • Willing to continually update skills and knowledge regarding healthcare procedures.
  • Ability to maintain patient confidentiality.
