As part of a multidisciplinary public health team, the Community Health Worker (CHW) is responsible for promoting the health and well-being of communities disproportionately affected by HIV. The CHW conducts targeted outreach, provides HIV/HCV/syphilis screening, delivers health education, connects individuals to PrEP and other prevention services, and ensures linkage and re-engagement in HIV care for people who are newly diagnosed or lost to follow-up. The focus population includes people vulnerable to HIV acquisition and individuals unaware of their HIV status.
