Community Health Program Manager – Southside Regional Medical Center Job Summary: The Community Health Program Manager oversees the implementation and evaluation of one or more community health programs. They are responsible for ensuring project goals are met, budgets are followed, and resources are coordinated effectively. Through partnering with internal and external stakeholders, they play a key role in community engagement and collaboration.

Requirements

  • Bachelor’s degree in relevant field, or equivalent experience (required)
  • 3 years related experience (required)

  • Certified Program Management Professional (PgMP) Project Management Institute (preferred)

Responsibilities

  • Oversees implementation of program(s) meant to improve the health, economic, social, and/or environmental quality of life in a community.
  • Responsible for overall program execution and oversight of performance outcomes.
  • Identifies potential beneficiaries of the program and, as needed, establishes and maintains relationships with the goal of encouraging use of the organization's programs.
  • Builds and maintains mutually beneficial relationships with local business groups, community representatives and organizations, public agencies, schools and not-for-profit organizations.
  • Implements and monitors performance metrics to demonstrate achievement of program objectives and ensure compliance.
  • Contributes to data collection, reporting, and/or continuous quality improvement efforts for programs.
  • Collaborates with community health leadership on approaches for addressing complex/sensitive community issues, targeting locations for initiatives, etc.
  • May assist in facilitating CHNA and Community Health Improvement Plan (CHIP) process.
  • May manage events and coordinate community outreach to promote the organization’s services in a positive and community-oriented way.
  • Ensures grant compliance and completes grant reports, as needed.
