Job Summary The Specialist is a key member of Mass General Brigham’s Health Corps, a dedicated community-based workforce advancing the system’s Community Health Strategy. Embedded in MGB’s priority communities, Health Corps members help individuals and families access care, address health-related social needs, and manage conditions that contribute to health disparities. This position focuses on outreach, education, and navigation related to two of MGB’s identified priority conditions: colorectal cancer and cardiometabolic disease. Community Health Workers provide blood pressure screenings, deliver basic health counseling, and support individuals in connecting to services that promote prevention, early detection, and long-term health. This role requires deep familiarity with the strengths, cultures, and needs of the communities served, which include Chelsea, Revere, Dorchester, Roxbury, Mattapan, Mission Hill, Hyde Park, Jamaica Plain, Roslindale, Lynn, Salem, and Waltham.
