About The Position

The Community Health & Engagement Manager is a detail-oriented self-starter with a dual passion for promoting community health and health equity and guiding staff across the HHC system related to reporting, data quality, and use. The Community Health & Engagement Manager serves as a central system resource to effectively and efficiently manage platforms related to community health and community benefit across all HHC regions. With oversight of the HHC community benefit platform (CBISA) and HHC’s Connections that Matter platform, as well as direct participation in HHC’s health needs assessment and community engagement initiatives, the Community Health & Engagement Manager works with multidisciplinary teams to achieve results and drive authentic community engagement.

Requirements

  • High School Diploma and 6+ years of relevant experience in lieu of college degree
  • Over 5 years of experience in community health, public health, social services, human services, and/ or health care
  • A community perspective and knowledge of nonprofits and community-based organizations
  • Comfort connecting with and relating to people from a broad range of backgrounds; ease in conversation and facilitation
  • Operations and / or project management experience, with an emphasis on gathering input, defining a clear path, engaging others, and taking action to achieve goals
  • Strong writing, presentation, and communication skills; demonstrated ability to communicate effectively with a broad range of stakeholders
  • Fluency with databases, data entry, and reporting
  • Effective computer skills (PC preferred with Microsoft Office – Word, Excel, PowerPoint)
  • Ability to think systemically and create guidance and processes; ability to engage others in understanding and following processes
  • Experience coaching others about data entry, data quality, and use of data (CBISA, Findhelp, and other platforms)
  • Ability to identify learnings and translate them into user-friendly information that is relevant for diverse stakeholders.

Responsibilities

  • Oversight of CBISA database and reporting across all HHC regions
  • Coach and train colleagues entering data into CBISA, setting and maintaining standards for quality data entry and use
  • Support and improve system’s capability to perform, identify, capture, and report community benefit activities
  • Research and maintain knowledge of successful strategies for communicating community benefit and social impact; analyze applicability and/or replicability to Hartford HealthCare
  • Draft and socialize CBISA reporting standards and goals across the enterprise
  • Translate community benefit data and trends into valuable insights system leadership
  • Serve as central resource and convener for regional community health and engagement teams, including leading consistent community health and engagement team meetings
  • Support relationships and engagement with community advisory group (Health Equity Champions) to routinely gather feedback and input, including leading quarterly meetings
  • Manage the Health Equity Innovation Fund, including sending out applications, collating application scoring, facilitating discussions with community and colleagues related to grantee selection, and ongoing relationships with grantees
  • Actively participate in convenings and coalitions that serve to promote the integration of the clinical and community health systems
  • In partnership with FindHelp, support the expansion and maintenance of the Connections That Matter online social resource platform
  • Develop necessary relationships, programs and educational materials to maximize the use of Connections That Matter in the clinical workflow
  • Work with key internal and external stakeholders to educate and establish goals around the use of the platform
  • Identify areas for growth and opportunity related to social needs screening and connections to resources, as well as needed resources and project plans needed for success
