About The Position

Under the Special Diabetes Program for Native Americans grant program funded by the Indian Health Services (IHS), the Community Health Educator will function as an integral member of the healthcare team at SDAIHC. SDPI’s purpose is to provide diabetes treatment and/or prevention services and activities for American Indian/Alaska Native (AI/AN) communities to reduce risk factors for diabetes and diabetes-related conditions. The Community Health Educator supports the Special Diabetes Program for Native Americans (SDPI) by conducting community outreach and delivering health education activities that encourage participation in diabetes prevention and wellness programs. This position focuses on building trusted relationships with community members, promoting SDPI services, and increasing awareness of healthy lifestyle practices. The Community Health Educator works closely with program staff and community partners to engage participants, support group education activities, and contribute to program implementation and reporting in alignment with Indian Health Service (IHS) SDPI goals. The candidate must be able to demonstrate knowledge and skills necessary to perform all job-related activities as outlined below.

Requirements

High school diploma or equivalent required.
Bachelor’s degree in health education, public health, community health, health promotion, or a related field OR an equivalent combination of education and relevant community health or outreach experience.
Has a clean driving record and insurance as required by the state.
Reliable transportation.
CPR/ BLS certification: Maintain a current Basic Life Support (BLS) certification issued by the American Heart Association (AHA), the American Red Cross, or an equivalent organization. Certification must include an in-person, hands-on skills assessment. Online-only certifications are not accepted.
Annual background checks: Consent to annual background checks as a condition of continued employment, to ensure compliance with organizational standards and eligibility requirements.
For-Cause Drug Screening: Comply with drug screening requirements when initiated by the organization for cause, to support a safe, compliant, and drug-free workplace including marijuana.
Ongoing Compliance Requirements: Maintain up-to-date compliance with all required annual renewals, including professional licenses, certifications, physical examinations, TB testing, and mandatory regulatory trainings as assigned by the San Diego American Indian Health Center (SDAIHC).

Nice To Haves

Experience serving a multicultural population, American Indian/Alaska Natives in particular.
FQHC and/or public health background.
Familiarity with Community Health Clinics and/or Indian Health Clinics.
Familiarity with grant-funded program documentation and reporting
Knowledge of local community culture, traditions, and resources

Responsibilities

Conduct proactive, in-person outreach to American Indian/Alaska Native community members at community sites, events, partner locations, and SDAIHC programs to promote SDPI services and activities.
Build and maintain trusted relationships with community members, elders, families, and partner organizations to increase awareness and participation in diabetes prevention and wellness programs.
Represent SDAIHC and SDPI professionally at health fairs, community meetings, cultural events, and outreach activities.
Identify community needs, barriers to participation, and opportunities for engagement; communicate findings to SDPI leadership for program improvement.
Assist in the planning, coordination, and delivery of culturally appropriate diabetes prevention and wellness education activities, workshops, group classes, and community presentations.
Support group education sessions by preparing materials, assisting facilitators, managing attendance, and engaging participants.
Reinforce key health education messages aligned with SDPI goals, evidence-based practices, and IHS guidance.
Ensure educational activities are culturally respectful, trauma-informed, and accessible to diverse community members
Accurately collect and maintain participant data, outreach encounters, attendance logs, sign-in sheets, and other required SDPI documentation.
Enter and track outreach and education activities using approved SDAIHC systems, tablets, spreadsheets, dashboards, or reporting tools.
Support timely and accurate reporting to meet IHS SDPI grant requirements, internal monitoring, and audit readiness.
Review outreach and participation data with program leadership to identify trends, gaps, and areas for improvement.
Work collaboratively with SDPI staff, Health Promotion staff, clinical teams, and other SDAIHC departments to support integrated program delivery.
Coordinate outreach and education efforts with tribal programs, urban Indian organizations, schools, and community partners.
Participate in SDPI meetings, trainings, and planning sessions as assigned.
Support cross-program collaboration when appropriate, while maintaining SDPI scope and funding compliance
Collaborate with the Health Promotion Manager to develop and implement outreach strategies that support SDPI goals and deliverables.
Assist with culturally appropriate social media and community education campaigns related to prevention of diabetes and wellness.
Provide feedback on outreach effectiveness and recommend adjustments to strategies to improve outreach and participation
Maintain strict confidentiality of participant information in compliance with HIPAA, SDAIHC policies, and grant requirements.
Ensure all outreach and education activities comply with IHS SDPI guidelines, SDAIHC policies, and applicable regulations.
Maintain accurate, organized, and audit-ready records at all times.
Complete required trainings and certifications as required by SDAIHC and SDPI
Conduct community outreach up to three (3) days per week for up to six (6) hours per day, including evenings or weekends as needed.
Document outreach activities and results in a timely manner following each field-based activity.
Travel to community sites using reliable transportation as required
Perform other related duties as assigned to support SDPI program objectives and organizational needs
