The Community Health Ambassador (CHA) supports the development and implementation of effective outreach strategies for Wellness Center Program services, activities, and events. Working alongside the Outreach & Engagement Coordinator and the broader Community Wellness Program (CWP) team, the CHA helps promote and implement initiatives that advance health equity, prevent chronic disease, and enhance holistic physical and behavioral health. This role is part of a collaborative partnership with the Department of Public Health and HOPE SF, focusing on families living in the Potrero Hill, Hunters View, Alice Griffith, and Sunnydale public housing communities.

Requirements

  • Must be at least 18 years old.
  • High school diploma, GED, or actively working toward GED completion.
  • Availability to work Monday--Friday, with weekends and evenings as needed.
  • Must live within the Hope SF community served in this role.
  • Able to enroll in and complete the City College Community Health Worker Certification Program within two years of hire.
  • Strong written and verbal communication skills.
  • Experience working collaboratively with community organizations, public agencies, or other stakeholders.
  • Ability to work effectively as part of a multidisciplinary team.
  • Strong organizational skills with the ability to follow through on tasks.
  • Ability to manage multiple projects and responsibilities.
  • Ability to work effectively with diverse populations across various backgrounds and experiences.
  • Ability to exercise discretion and independent judgment.
  • Professional, approachable, and adaptable demeanor.

Nice To Haves

  • At least one year of experience developing and implementing outreach strategies.
  • Proficiency in a second language (Spanish, Cantonese, or Samoan preferred).

Responsibilities

  • Promote and implement programs that focus on health equity, chronic disease prevention, and holistic physical and behavioral health.
  • Provide culturally relevant wellness programs tailored for individuals and families.
  • Improve holistic health, with an emphasis on heart and behavioral health, by increasing opportunities for physical activity, healthy eating, stress reduction, and social connection.
  • Co-create environments that make healthy choices easier and more accessible.
  • Engage with residents in public and subsidized housing to support their health and well-being.
  • Collaborate with community-based organizations, public health agencies, and city programs to enhance community health initiatives.
  • Collect and manage data at the community level to assess program effectiveness.
  • Assist in the development and implementation of outreach strategies to engage community members.
  • Ensure compliance with confidentiality policies, HIPAA regulations, and mandated reporting laws.
