About The Position

LifeLong Medical Care is seeking Community Engagement Specialists to join our Community Outreach & Engagement team. The Community Engagement Specialist will be responsible for building relationships with community members in Alameda and Contra Costa counties and contributing directly to increased healthcare enrollment and patient coverage numbers throughout the year. LifeLong recently created its Stability & Engagement Workgroup (SEW) which includes a series of initiatives. This role is part of the SEW Coverage Initiative. The roles and responsibilities for this role will support the SEW Coverage Initiative in limiting coverage loss among LifeLong patients who are considered at risk and will support the enrollment team in achieving at least 400 renewals and 450 new enrollments each year. To achieve these objectives the specialist will be responsible for coordination and participation in community events. They will also provide and coordinate enrollment training and education sessions for low-income individuals and families in Alameda and Contra Costa counties. Training and education programs could include group sessions and train the trainer sessions provided by health promoters/peer educators and other community health workers. This is a full time, benefit eligible, in-person position. Bilingual English/Spanish highly preferred. Please include a cover letter as part of your application. Requirements

  • Associates or Bachelor’s Degree in public, mental, and/or medical health education, social work, psychology, health education, nursing, education or communications. Related work experience is also an acceptable qualification.
  • Two (2) years (24 months) of conducting presentations, seminars or closely related programs and/or providing health education experience
  • Must be able to work some evenings and weekends
  • Must have a reliable source of transportation to travel within Contra Costa County and Alameda County

Nice To Haves

  • A BS or MS degree in nutrition, health education or related field is preferred.
  • 2 years of experience in planning and implementing of community programs.
  • Knowledge of Bay Area low-income and diverse racial/ethnic communities.
  • Ability to work cooperatively and independently.
  • Strong interpersonal and communication skills.
  • Capable of both professional and personal interactions with wide variety of health professionals in a culturally diverse environment.
  • Bilingual Spanish preferred.
  • Excellent organizational skills, ability to prioritize projects and follow through on detail with a high level of accuracy.

Responsibilities

  • The Community Engagement Specialist is a team member who works closely with the Community Engagement Project Manager and entire Community Engagement team to meet the coverage deliverables outlined in the SEW Coverage Initiative.
  • The Community Engagement Specialist will participate in outreach at schools, churches, local government and community agencies.
  • Must be able to provide their own transportation, maintain accurate and timely records, including designated information on participants, and produce reports on all program activities as assigned.
  • The Community Engagement Specialist will also be responsible for leading training and education sessions on the importance of coverage, be an excellent communicator (written and verbal).
  • This role will collaborate closely with the enrollment team and the communications team.
  • Participation as a team member with various community partners and the ability to maintain professional competence through on-going trainings and professional development opportunities is essential to this position.
