About The Position

The Community Engagement Manager plays a key role in the Pathways to Success Program by ensuring that youth and families are connected to local community resources that support their stability during and after enrollment in services. This role strengthens the connections between the Care Coordination and Support Organization (CCSO), referral sources, and community partners.

Requirements

  • Strong knowledge of local community resources and social service systems
  • Experience in community outreach, partnership development, or social services preferred
  • Excellent communication and relationship-building skills
  • Ability to collaborate across teams and with external stakeholders
  • Organized and self-motivated, with strong follow-through
  • The ability and willingness to actively participate in a fast-paced, multidisciplinary team environment.
  • Must meet DCFS/CANTS Clearance Requirements.
  • Must be at least 21 years of age.
  • Must possess and maintain a valid driver's license and maintain proof of automobile insurance.
  • High School diploma or equivalent required
  • Completion of CARE and TCI training is an internal requirement for all staff and must be completed upon hire.
  • Ongoing application of these principles is expected and essential to successful job performance.
  • Pass the written test annually
  • Pass the physical test every six months (if applicable to their role)

Nice To Haves

  • Bachelor's degree in social work, human services, or related field is preferred.

Responsibilities

  • Identify and collaborate with local and regional community networks to expand access to available resources and supports for youth and families.
  • Ensure Care Coordinators are informed about how to access and navigate community networks.
  • Maintain and update a comprehensive Community Resource Directory, ensuring it reflects current and relevant services.
  • Participate in community network meetings to promote awareness of CCSO services among community partners.
  • Build and maintain relationships with referral sources and service providers to support coordinated care for enrolled youth.
  • Facilitate quarterly meetings with community partners and service providers to; share updates on available services and support, identify service gaps and opportunities for improvement, and encourage collaborative problem-solving and partnership development.
