About The Position

Under the direction of the Director of Faith-Based and Community Engagement Initiatives, or designee, the Community Engagement Health Worker works as an integral member of behavioral/mental health programs within Genesee Health System. Work occurs in a variety of venues and locations, including but not limited to schools, Churches, healthcare facilities, community agencies, group and individual settings, community events, and canvasing. This individual will use the Community Health Worker model, as well as other evidence based crisis and trauma support methods. Primary responsibility will be to develop and maintain relationships with Faith-based organizations, government agencies, and community-based groups by creating effective communications strategies. Provides education, training, and support to individuals and families as well as makes referrals for other needed services and supports.

Requirements

  • High school diploma or equivalency AND Michigan CHW certification OR complete CHW certification within one (1) year of hire AND a minimum of one (1) year experience working with community based groups or organizations that focus on addressing social needs and concerns.
  • Employee must maintain Community Health Worker certification to retain employment.
  • Must be willing and available to work regularly scheduled hours, including some weekends and evenings (e.g. attending Resource and Health Fairs and other events that may occur after regular working hours including some weekends).
  • Must have a valid Michigan driver’s license, current automobile insurance, and own transportation or the use of a vehicle during working hours.
  • Must be able to physically perform job related tasks.
  • Must be willing and able to access services in their home (working remotely), in the community and office settings.
  • Must be willing to obtain Chauffeur’s Driver’s License if requested.
