About The Position
As a Mobile Health Community Health Worker, you’ll connect patients to care and community resources—helping address social determinants of health and improve overall well-being. You’ll work closely with clinical teams and community partners to support patients through education, outreach, and care coordination.
Requirements
- High School diploma required; Associate degree preferred
- CHW certification or related experience a plus
- Strong communication and community engagement skills
- Valid driver’s license and reliable transportation
- Experience with diverse or underserved populations preferred
Responsibilities
- Provide health education and outreach in community settings (homes, schools, clinics, etc.)
- Connect patients to community resources and support services
- Assist with care coordination, referrals, and follow-ups
- Document services in electronic medical records
- Support telehealth visits and care team initiatives
- Advocate for patients and help navigate healthcare and insurance systems
