About The Position

The Community Engagement Coordinator – Cal AIM is a community-based outreach and engagement role focused on identifying, educating, and connecting eligible Students to Enhanced Care Management (ECM) and Community Supports and responsible for the outreach and coordination, improving health outcomes, enhancing satisfaction, and reducing unnecessary healthcare utilization services under California’s Cal AIM initiative. This position serves as a front-end engagement and referral liaison and does not carry an ongoing caseload. The coordinator is responsible for conducting outreach, completing initial screenings and assessments, building and maintaining relationships with community partners, representing Amity at resource fairs and community events, and facilitating warm handoffs to ECM Care Management and internal Amity programs. The role supports early identification of Cal AIM-eligible individuals particularly those experiencing homelessness, justice involvement, serious mental health or substance use needs, or high social complexity and helps ensure appropriate linkage into ECM, Community Supports, or other Amity services.

Requirements

  • Bachelor’s degree in social work, psychology, public health, or related field; or Associate’s degree and minimum of two years’ experience in client-related field, providing case management and community outreach. OR The equivalent of a high school diploma and three cumulative (3) years of experience in providing outreach, substance abuse counseling, reentry services, and case management within a culturally diverse population.
  • Strong communication and interpersonal skills, capable of building relationships with students, families, team members, and community partners.
  • Understand various ethnic and socioeconomic backgrounds within a multicultural environment.
  • Knowledge on how to administer assessments, substance abuse counseling, reentry services, and case management within a culturally diverse population, preferably with both men and women.
  • This position requires local travel and the ability to navigate public transportation with students.
  • Required to use company vehicle as well as personal vehicle to visit county jails and transport clients to and from appointments and other locations as needed.
  • Required to have car insurance, vehicle, and valid driver’s license

Nice To Haves

  • Bilingual, fluent Spanish/English oral and written skills are highly desirable.
  • Community Health Worker certification through California’s HCAI Department.

Responsibilities

  • Conduct outreach to identify and engage eligible in ECM services, using multiple strategies and modalities for engagement.
  • Triage and outreach community members that are referred by MCP/Member Information File and all other computing pathway referrals.
  • Interview and conduct assessments of students’ needs, including medical, behavioral, and social factors.
  • Conduct proactive, in-person outreach in community settings, including reentry programs, shelters, parole offices, treatment programs, and community-based organizations.
  • Represent Amity at community resource fairs, coalitions, and outreach events to educate stakeholders and potential participants on ECM and Community Supports.
  • Deliver presentations and informational sessions on CalAIM, ECM, and Community Supports to community partners and referral sources.
  • Build and maintain strong working relationships with external partners, referral agencies, managed care plans, and county stakeholders.
  • Coordinate care across multiple providers and services, ensuring seamless integration of medical and social care.
  • Act as a student advocate, ensuring the student’s needs and preferences are central to care decisions.
  • Conduct initial screenings and needs assessments to determine potential eligibility for ECM and/or Community Supports programs.
  • Verify and document eligibility indicators related to CalAIM populations of focus.
  • Support coordinated transitions from outreach to enrollment without maintaining ongoing case management responsibility.
  • Serve as a liaison between community partners, ECM teams, Community Supports providers, and other Amity programs.
  • Document services in the agency’s Electronic Health Record within 72 hours of each service and in compliance with agency policy, as well as contractual, state, and federal regulations.
  • Track outreach activities, community contacts, and referral outcomes.
  • Maintained concise and comprehensive case records on all participants seeking services.
  • Conduct Outreach and community engagement to educate and engage members of community on ECM Services
  • Provide accurate daily and monthly documentation and case notes as per funding source guidelines.
  • Attend workshops, meetings, and trainings as requested by supervisor.
  • Provide required daily, weekly, and monthly reports related to outreach, engagement, and referrals.
  • Performs other related duties as assigned.
