About The Position

The Community Engagement Ambassador & Program Navigator serves as a trusted liaison between the Springfield Urban League (SUL) and priority populations within high-need communities. This contractual role focuses on grassroots outreach, health education, and navigation services designed to reduce barriers to care and improve access to health screenings, clinical services, and supportive resources. Ambassadors play a critical role in advancing health equity by fostering trust, addressing social determinants of health, and ensuring individuals are successfully linked to appropriate care and follow-up services.

Requirements

  • High school diploma or equivalent required; additional training or experience in public health, social services, or community outreach preferred.
  • Strong ties to or familiarity with the communities being served.
  • Excellent interpersonal, communication, and relationship-building skills.
  • Ability to work independently and conduct outreach in community-based settings.
  • Demonstrated understanding of health equity, social determinants of health, and community-based service delivery.
  • Basic computer skills and ability to complete required documentation and reporting.
  • Willingness to work flexible hours, including evenings or weekends as outreach demands require.

Nice To Haves

  • Prior experience as a community health worker, navigator, ambassador, or peer educator.
  • Experience working with underserved or marginalized populations.
  • Knowledge of local health systems, community resources, and referral networks.

Responsibilities

  • Conduct grassroots outreach in high-priority neighborhoods identified by SUL and funding partners.
  • Serve as a primary point of contact between community members and SUL Health Initiatives programs.
  • Distribute health education materials related to preventive care, screenings, and available community resources.
  • Identify and engage individuals in need of health screenings, navigation services, and referrals.
  • Provide one-on-one navigation support to help clients overcome barriers to care, including but not limited to: o Transportation challenges o Appointment scheduling o Fear of stigma or discrimination o Childcare needs o Employment or job-training barriers
  • Facilitate linkages to clinical providers, social services, and community-based support organizations.
  • Assist with follow-up to ensure successful connection and retention in care.
  • Accurately document outreach activities, client interactions, and referrals in accordance with grant and program reporting requirements.
  • Participate in trainings, team meetings, and capacity-building activities as required.
  • Represent Springfield Urban League in a professional, culturally responsive, and community-centered manner at all times.
