About The Position

The Community Case Manager Supervisor plays a critical role in supporting vulnerable adults by overseeing high-quality, community-based case management services. This position bridges gaps in care for individuals experiencing serious mental illness, aging-related challenges, or increased vulnerability due to abuse, neglect, or exploitation. This role provides clinical oversight, guidance, and support to case management staff while ensuring services are person-centered, strengths-based, and aligned with regulatory and quality standards. The supervisor works closely with community partners, caregivers, and the Department of Children and Families (DCF) to promote stability, safety, and improved quality of life for those served.

Requirements

  • Bachelor’s degree in human services or related field, or equivalent combination of education and experience
  • Strong communication, organization, and problem-solving skills
  • Ability to remain composed, flexible, and solutions-focused in a fast-paced environment
  • Valid Kansas driver’s license and insurability through Prairie View
  • Ability to complete required trainings within designated timeframes
  • Proficiency with basic computer and documentation systems
  • Must pass required background screenings (KBI, Central Registry – adults and children)

Nice To Haves

  • Experience working with individuals experiencing serious mental illness, older adults, or vulnerable populations preferred
  • Supervisory or leadership experience strongly preferred

Responsibilities

  • Provide supervision, guidance, and support to Community Case Managers delivering services in the community
  • Ensure services emphasize participant strengths, autonomy, and individualized goal-setting
  • Support staff in engaging participants with complex mental health, physical health, and social needs
  • Provide consultation on crisis intervention, problem-solving, and care coordination
  • Promote ethical, trauma-informed, and culturally responsive practices
  • Support coordination of services related to housing, finances, healthcare, daily living skills, and natural supports
  • Collaborate with internal teams, DCF, caregivers, and external agencies to reduce vulnerability and improve outcomes
  • Assist staff in navigating complex systems to connect participants with appropriate resources
  • Ensure timely, accurate, and compliant clinical documentation
  • Monitor completion of required reporting and assessments (e.g., AIMS)
  • Support staff in maintaining organized schedules and managing competing priorities
  • Participate in team meetings, clinical supervision, and administrative supervision
  • Support adherence to productivity expectations and billable service requirements
  • Participate in performance improvement initiatives and quality assurance activities
  • Assist in monitoring compliance with organizational, state, and program standards
  • Provide coaching, feedback, and performance guidance to staff
  • Support onboarding, training completion, and ongoing professional development
  • Foster a positive, collaborative, and respectful team environment
  • Model professionalism, accountability, and ethical conduct
  • Ensure client information is handled with discretion and full compliance with HIPAA and confidentiality standards
  • Promote professional boundaries and ethical decision-making
