About The Position

ABOUT THE ROLE The Community-Based Therapist plays a key role in supporting youth as they transition from institutional care into family or family-like settings. Working primarily in the community including family homes, foster homes, group homes, schools, and other natural settings this role coordinates services and planning across systems to promote stability, connection, and long-term success. Therapists partner closely with youth, caregivers, and multidisciplinary teams to ensure everyone is aligned around shared goals, while building meaningful, culturally responsive relationships that honor each family’s values, strengths, and lived experiences. ABOUT TOP The Therapeutic Outpatient Program (TOP) provides Specialty Mental Health Services (SMHS) that enhance the Contra Costa County continuum of care, ensuring that culturally-responsive mental health services for youth with attachment or permanency-related needs are family-driven and informed by a deep understanding of trauma. TOP is designed to include seamless service continuity, close collaboration and coordination with other existing services within the Contra Costa County system of care, flexible to scale up services over time according to youth, family, and county needs, and the ability to provide services in community setting during flexible hours to promote client engagement and effectively meet their individualized service needs. Requirements

  • Master's degree in social work, marriage and family therapy or psychology
  • Registered or eligible to register with the California BBS required
  • Experience working with children in out-of-home care preferred
  • Own vehicle, Valid California Driver's License required
  • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements
  • At least 21 years of age

Responsibilities

  • Meet regularly with youth and caregivers in community settings to advance treatment goals using culturally responsive, evidence-informed practices
  • Coordinate care and service planning with multidisciplinary teams to support safe, stable transitions for youth and families
  • Collaborate with caregivers, agency staff, schools, county partners, and other providers to develop and implement individualized care plans, behavioral supports, and interventions
  • Complete comprehensive assessments and evaluations to inform treatment planning and service delivery
  • Provide clinical consultation and case planning support to team members, as appropriate
  • Build and maintain strong relationships with community partners and natural supports to enhance continuity of care
  • Maintain accurate, timely documentation including progress notes, care plans, outcome measures, and incident reports in compliance with program and Medi-Cal requirements
  • Participate in crisis response and intervention services, including on-call rotation when applicable
  • Comply with all mandated reporting requirements and applicable Medicaid/Medi-Cal standards
  • Maintain strict confidentiality of client and agency information
  • Engage in ongoing self-reflection, professional development, and organizational efforts related to diversity, equity, and inclusion
  • Utilize crisis communication and de-escalation techniques as per Seneca's crisis response training This may include intervening with the youths’ behavioral program through physical management
