About The Position

The Community Based Case Manager at Clarvida is a part-time role focused on delivering behavioral and mental health services to clients in their homes and communities. The position involves working with clients aged 5 through adult who face various behavioral issues and mental health challenges. The Case Manager is responsible for maintaining documentation, conducting weekly meetings with clients, and providing support to foster independence and improve quality of life.

Requirements

• Bachelor's degree in a Human Service field (Social Work, Psychology, Sociology, Counseling, etc.)
• Valid driver's license with reliable transportation and proof of car insurance.
• Ability to pass background checks, including Enhanced Fingerprinting with ID DHW.
• Experience utilizing electronic health records system is a plus.

• Experience in discharge planning
• Knowledge of community resources for behavioral health services.

Responsibilities

• Deliver services to clients in their home and community focusing on individual behavioral and mental health needs.
• Maintain documentation including daily progress notes, treatment plan updates, and discharge summaries.
• Meet with clients weekly to focus on skill building and access to community resources.
• Provide coping skills to clients to foster independence and improve their quality of life.
