About The Position

The Community Based Case Manager is responsible for: Developing and implementing Individual Service Plans (ISP). Supporting community inclusion and independent living skills. Providing care coordination, direct care, and supervision as needed—including personal care and daily living activities. This role aligns with the Charles River Center's values of self-direction and follows our established principles, policies, and procedures.

  • Bachelor’s degree (or currently enrolled) in Human Services or a related field
  • Equivalent experience may be considered in some cases
  • Fluent in English with the ability to read and interpret documents.
  • Ability to communicate effectively with clients, colleagues, families, and community members.
  • Strong written and verbal communication skills for reports, correspondence, and presentations.
  • Employee must have significant endurance to sustain the physical demands of the role.
  • Regular activities include sitting, standing, walking, stooping, kneeling, or crouching.
  • Frequent use of hands/fingers to handle objects and tools.
  • Ability to generate force to maneuver up to 100 pounds (assistance provided when possible).
  • Must be able to lift up to 30 pounds from floor to waist and 10-15 pounds from waist to overhead.
  • Ability to follow detailed written and oral instructions.

Nice To Haves

  • Experience working with individuals with developmental disabilities
  • Familiarity with behavior modification techniques
  • Strong oral and written communication skills
  • Valid Massachusetts Driver’s License and reliable personal transportation
  • Proficient in Microsoft Office and general computer use

  • Support healthy, culturally appropriate meal planning and purchasing
  • Provide transportation and skill-building in grocery shopping
