About The Position

Under the Supervision of the Program Director or their designee, the Community Based Care Coordinator shall be responsible for person centered, strengths-based case management services for individuals seeking treatment services. The Community Based Care Coordinator shall operate in an ethical, professional manner, interact with others in such a fashion as to create and maintain respect for the agency, its partners, and its stakeholders. The Community Based Care Coordinator will be part of a strong multidisciplinary team, working to serve our clients and communities. The Community Based Care Coordinator will assist clients with developing person-centered goals (finding jobs, housing, developing hobbies, building supports) while maintaining a healthy lifestyle.

Requirements

  • Must be certified by the Kansas Department of Aging and Disability Services as a Person-Centered Case Manager prior to beginning employment in this position.
  • High school graduate or equivalent.
  • Ability to navigate EHR
  • Minimum of one-year paid experience providing case management or workforce development services to a similar population.
  • Excellent interpersonal skills required as well as ability to communicate effectively both verbally and in writing.
  • Capable of maintaining a team spirit and the ability to work cooperatively with others.
  • Must be adaptable to change and have effective organizational skills.
  • Must be able to organize and follow through on multiple tasks.
  • Display consistent, good judgment and decision-making skills.
  • Ability to establish and maintain rapport with target population.
  • Must be able to pass background/security check.
  • Employees who have been diagnosed with any type of substance use disorder or behavioral addiction/compulsion must have sustained a minimum of two consecutive years of abstinence from all intoxicating substances and symptoms of their chemical and/or behavioral addiction; and must be in full sustained remission.
  • Must be proficient in Microsoft Office Word. Outlook and with a basic understanding of Excel preferred.
  • Position may also involve bending, walking stairs, and lifting up to-40 pounds on occasion.

  • Although not mandatory persons that are bilingual (English & Spanish) will be given strong consideration.

Responsibilities

  • Respond to referrals from Kansas Highway Patrol (KHP) personnel or other first responders, involving individuals suspected of having substance use disorders.
  • Provides case management and referral services for program participants in the following basis areas of human need as required: employment, vocational training, continued education or GED testing, community resources, legal assistance, housing, family enrichment, family violence, and other client needs.
  • Provide training to KHP and other first responders on substance use disorders and how to initiate a referral.
  • Tracks and collects pertinent data for grants; and reports and completes all required reports and summaries in a timely manner for the Program Director, KHP, and the Sunflower Foundation.
  • Must attend and participate in all staff meetings.
  • Maintain paperwork, and reporting outcomes in a timely manner.
  • Facilitate and make appropriate program participant referrals to community-based services related to their personal needs consistent with Mirror policy and guidance from the grantor.
  • Work with KHP to develop a shared outcomes dashboard.
  • Ensure any public communications regarding the project are coordinated with Mirror management and KHP. Ensuring that all communications acknowledge the Kansas Fights Addiction program.
  • Perform other job responsibilities as assigned by the Program Director or members of the Mirror Management Team.
