At MDH Network, we serve as a source of support and guidance to empower and improve the lives of Individuals with intellectual disabilities, developmental disabilities, behavioral challenges, dual diagnoses, or forensic involvement through person-centered planning. With over 30 residential facilities and several supportive, non-residential programs located throughout Southern California, we strive to support Individuals to live their best lives. Our Community Integration Program, Inc. is seeking a compassionate and reliable Community Activities Specialist (CAS) to support Individuals enrolled in our In-Home Day Program. This role is ideal for professionals experienced in developmental disabilities services who are committed to person-centered, community-based support. The Community Activities Specialist provides structured, individualized support to Individuals in accordance with their Individual Program Plans (IPP) and Individual Service Plans (ISP). The CAS supports community integration, skill development, and independence while ensuring compliance with HCBS, Title 17, and Title 22 regulations. We are seeking a professional who is committed to the mental health and developmental services sector and skilled in working directly with Individuals to provide support in areas such as social skills, budgeting, self-advocacy, work habits, and community access.
