An overview of this position entails supporting key actions of the Communities that Care (CTC) project. This position focuses on engagement of parents, youth and community agencies to help build capacity and impact of the overall project. Additionally, this position obtains all referrals for the Individual Services and Staff Team (ISST), facilitates the ISST meetings, works directly with families through the ISST process and provides case management, as needed.

  • Bachelor's degree in human services or related field, or four years of related work experience; or equivalent combination of education and experience.

Responsibilities

  • Provides direct support to the Collaborative Coordinator of the Communities that Care project (CTC) and Collaborative Management Program (CMP) through direct work with families, youth, parents, grandparents and community partners
  • Represent CTC and CMP in multiple settings in the community.
  • Communicate with partner agencies and natural supports to arrange meeting logistics.
  • Administer any required assessments.
  • Participate in on-calls and facilitate ISST meetings.
  • Short-term case management and maintain required documentation.
  • Provide direct case management to families involved in CMP; support case leads while conducting case management.
  • Upon enrollment to CMP, create a family working file with all necessary documentation, assist families in developing their individualized goal/plan.
  • Enter and maintain data as required by assigned program.
  • Works with schools, community agencies and neighborhoods to engage participants in the CTC project.
  • Attend all required trainings, including the Shared Risk and Protective Factor Conference.
  • Support the Collaborative Coordinator by building and maintaining community relationships by attending appropriate meetings within the community, developing presentations and presenting/facilitating to the community as requested or required, and communicating with Partner agencies to ensure program goals are met.
  • Ensure an equitable approach is carried out by coordinating translation and interpretation services for meetings and CTC coalition related events.
  • Become trained in all required frameworks and best practices of CTC and CMP; including Positive Youth Development.
  • Consistently and intentionally represents, models and operationalizes Hilltop’s mission and values; actively works to integrate them with their own.
  • Aligns personal goals and daily activities with the visions, values, and strategies of the organization, assigned programs, and Regional Services team.
  • Other duties as assigned.
