About The Position

Traditions Home Health Services is currently seeking compassionate, skilled, reliable, and experienced Home Health Aide and/or CNA for the Stoughton area to assist with our senior clients who are in need of support with Activities of daily living and household support and companionship. Shifts Available: Daily 4-Hour Shifts example: 8am-12pm, 9am-1pm, 10am-2pm ect... 3pm-7pm, 4pm-8pm Day Shifts: example 8am-8pm, 10am-3pm (client dictates hours needed) Job Description: CNA or HHA would assist with the Activities of Living, personal care CNA or HHA would assist with light housekeeping CNA or HHA would encourage walking, exercising CNA or HHA would encourage stimulating conversation CNA or HHA would assist Activities and exercise according to the Care Plan CNA or HHA would encourage fluid intake, possible meal prep in accordance with orders/Care Plan Home Health Aide or Certified Nursing Assistant would competently documenting shift activities frequently during the shift digitally using a smartphone Home Health Aide or Certified Nursing Assistant would report any concerns to the Traditions Home Health Services Office Support Team in a timely manner We also have other wonderful clients that are in need of assistance for morning hours! 8am-12pm, 9am-12pm, 11am-2pm (examples) We are seeking caregivers to work with our clients who are Veterans. Requirements

Home Health Aide Certification or CNA Certification
2+Years experience working with your Certification
Must be kind, compassionate, reliable, and skilled HHA
Must have experience working with Dementia
Clean driving record, ability to pass a CORI check
Current TB test, QuantiFERON Gold Test, or X-ray
Dementia care experience
Speak, read, write the English language clearly, and effective communication skills.
You must own a cell phone.
