About The Position

Manages the provision of patient care by an interdisciplinary team for hospice patients and families. Monitors expected level of care and documentation based on regulatory and agency standards. Manage the personnel functions and performance of an interdisciplinary team. Supervises Home Care CNAs and CNA Staffing Specialist. Make assignments and schedules to ensure that patient care needs are met. Ensures quality of care provided by assigned staff.

Requirements

  • Associates degree from an approved school of nursing required.
  • 2-3 years in progressively more responsible related work experience with one-year concurrent experience in home health/hospice.
  • Current Georgia and Alabama RN License, or valid eNLC compact RN license, required
  • BLS CPR certification required
  • Current CHPN certification required or must be obtained within one year of position placement.
  • Valid United States driver’s license with a good driving record required
  • Reliable personal transportation required
  • Influenza vaccine during influenza season (Nov. – March) required
  • Knowledgeable of Medicare Conditions of Participation for Hospice & Inpatient Hospice (Part 418.100), Medicaid regulations, TJC standards of Hospice & Inpatient Hospice, Georgia State Hospice regulations, and Columbus Hospice Policies & Procedures.
  • Understanding of and ability to guide others in the provision of aggressive hospice care and control of symptoms, particularly pain.

Responsibilities

  • Supervise and direct all patient care activities CNAs and CNA Staffing Specialist.
  • Assign new patients to the appropriate clinical staff and re-assign as needed to ensure coverage of patient care.
  • Supervise home care CNAs and CNA Staffing Specialist.
  • Review scheduling with CNA Staffing Specialist to ensure consistency and best utilization of CNAs. Be proactive and flexible with staffing to ensure the best care possible while remaining within budget.
  • Monitor and review CNA reporting in EMR system for accuracy and completeness. Monitor and review CNA mileage reporting for accuracy and legitimacy.
  • Monitor and review CNA timecard reporting for accuracy and completeness.
  • Perform annual supervisor evaluation on home team CNAs while the CNA is caring for a patient, in conjunction with the annual performance evaluation.
  • Ensure that CNAs receive supervisory visits by home team RNs every 14 days, and that supervisory visits are documented appropriately and timely; ensure that at least one of the supervisory visits every 14 days is in person, with CNA performing care during visit.
  • Exception: CNA supervisor will perform supervisory visits every 14 days for patients with Medicaid; may delegate to staff RN occasionally, but in all instances must ensure that supervisory visits are made in person and documented at the time of the visit no less often than every 14 days for Medicaid patients.
  • Conduct mandatory meetings with Home Team CNAs as often as deemed necessary – no less than twice per year.
  • Responsible for reviewing patient charts to ensure compliance with hospice CNA plan of care.
  • Attend IDG when necessary to provide input on any issues regarding patient CNA services.
  • Encourage team members to obtain skills and education in areas of weakness, and encourage CNA’s to take the hospice CNA certification exam after at least two years of service.
  • Maintain regular communication with all assigned staff.
  • Manage conflict, problem solve and de-escalate situations as they arise in relation to patient care and among staff.
  • Assist Team Manager with performance evaluations and disciplinary actions. Recommending disciplinary actions including termination, per policy (when applicable).
  • Update supervisor on routine basis regarding staff concerns and other pertinent issues.
  • Orient new assigned employees to the specifics of their duties.
  • Ensure that assigned team members are compliant with continuing education requirements. Identify educational needs of CNAs and provide one-on-one and group education as identified.
  • Acquire 15 continuing education hours per year.
  • Of the required 15 hours per year, at least 12 hours must be in hospice-specific topics offered by Columbus Hospice.
  • Identify own learning needs and participate in education opportunities to meet those needs.
  • Participate in performance improvement/quality improvement/compliance activities.
  • Participate in TJC accreditation processes, and surveys by regulatory and other accrediting agencies.
  • Demonstrate commitment to the mission and service excellence process by adhering to all facets of the Columbus Hospice Policies and Procedures.
  • Take all reasonable precautions to maintain privacy and non-disclosure of any confidential information about patients, other staff members, and the general operations of Columbus Hospice.
  • Always represent the company in a positive and professional manner with others, e.g., co-workers, management, vendors, and customers.
  • Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job.
  • When unsure of a directed task or any part of job responsibilities, proactively seek the assistance of immediate supervisor.
  • Ensure job responsibilities are met, including attendance requirements and prompt arrival at work. Assist with special projects, as requested.
