About The Position

Works under the supervision of Care Manager (RN or PT) to provide personal care, comfort measures and environmental services for patients (including those needing special precautions) and performs specified treatments. This position must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served, including knowledge of the principles of growth and development over the life span. The role involves promoting and maintaining a safe work environment for staff and customers, incorporating national patient safety initiatives. Reliable transportation is required for assigned home care/hospice visits. The Home Health/hospice Aide may be required to work weekends and holidays as census and staffing require.

Requirements

  • Certified Nurse Aid (CNA) - Virginia Department of Health Professions (VDHP)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days
  • Drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the job
  • Valid Drivers License Required

  • Prior Nurse Aide work experience in a home care setting

Responsibilities

  • Provides personal care, comfort measures, environmental services, and specified treatments for a specified number of patients as determined by department standards.
  • Assists with client's personal hygiene needs, ambulation, exercise, and with medications that are ordinarily self-administered (according to state regulations).
  • Reports any changes in the client's condition and /or needs to the assigned supervising Registered Nurse.
  • Provides nutritional support as needed or directed.
  • Provides other supportive tasks as assigned and/or specified by the case manager.
  • Performs duties consistent with Hospice Aide training and permitted to be performed under State Law.
  • Ensures availability of resources (equipment, instruments, supplies) necessary to ensure positive patient outcomes.
  • Recognizes emergency situations and responds competently.
  • Maintains clean, safe and orderly work/patient environment by cleaning work stations/areas, equipment, stocking supplies, and placing equipment in proper storage areas.
  • Assists and participates in training and support of all team members, volunteers and students.
