About The Position

JOB SUMMARY: A paraprofessional person who is specifically trained, competent and performs assigned functions of personal care to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN). Acts as an advocate for the Hospital and Home Care programs and adheres to the hospital Mission, Vision, Values and Motto of “The Patient is First in All We Do.”

Requirements

  • High school graduate.
  • Must have completed the CNA training course as outlined by the State of Utah, and have passed the State Office of Education Certified Nurse Aide Certification program.

Responsibilities

  • Understands and adheres to established Agency policies and procedures.
  • Performs personal care, bath and hands-on care as assigned.
  • Completes assigned logs and appropriate visit notes related to patient care in a timely manner according to agency policy.
  • Completes a weekly schedule of patients assigned and works closely with HHA Coordinator to assure patients are seen according to the plan of care.
  • Reports changes in the patient’s condition and needs to the RN or supervisor.
  • Reports to the office in a timely manner when unable to see patients due to illness or for other reasons.
  • Assists coordinator to cover patient care when planned time off is taken.
  • Willingly participates in required supervisory visits as required by law.
  • Performs household services essential to health care in the home as assigned.
  • Ambulates and exercises the patient as assigned.
  • Performs simple procedures as an extension of the therapy or nursing services, e.g., range of motion (ROM) exercises as assigned. HHA will demonstrate and verbalize knowledge and competency of the task prior to performing it independently.
  • Assists with medications that are ordinarily self-administered as assigned.
  • Attends in-service and continuing education programs as scheduled. Will complete at a minimum 12 hours of in-service training as assigned by the agency, and complete all Relias Learning training that is assigned including OSHA.
  • Demonstration of competency by passing a yearly written exam and skills pass offs as per Medicare regulations.
  • Attends and participates in patient care conferences as scheduled.
  • Practices infection precautions and appropriate cleaning of equipment. Observes aseptic technique as required.
  • Demonstrates a spirit of cooperation and enthusiasm in order to create a team atmosphere conductive to self-improvement and agency growth.
  • Performs other duties and responsibilities as assigned by the home health Clinical Manager or Nursing Supervisor, participating in quality improvement teams and agency programs.
  • Assists with PR activities such as BP clinics, health fairs, support groups, community education, etc.
  • Assists the patient in learning appropriate self-care techniques.
  • Participates in the on-call rotation schedule.
