About The Position

The Certified Nursing Assistant (CNA) provides direct, hands-on care and supportive services to adult participants in an Adult Day Health Care (ADHC) facility under the supervision of a Registered Nurse or Licensed Practical Nurse. This role assists participants with activities of daily living, comfort measures, and routine health monitoring while supporting a safe, structured, and therapeutic day program. The CNA works collaboratively with nursing and interdisciplinary staff to promote participant dignity, safety, independence, and quality of life in accordance with individualized care plans and applicable state and Medicaid regulations.

Requirements

  • Current Certified Nursing Assistant (CNA) certification in the state of practice.
  • High school diploma or GED required.
  • Experience in adult care preferred; experience in adult day health care, long-term care, rehabilitation, or home health preferred.
  • Current CPR/BLS certification required.
  • Knowledge of basic patient care, infection control, and safety practices.
  • Ability to follow care plans, instructions, and delegated nursing tasks.
  • Ability to meet background screening, training, and competency requirements per state and Medicaid regulations.
  • Strong communication, teamwork, and interpersonal skills.
  • Physical ability to perform job duties with or without reasonable accommodation.

Responsibilities

  • Provide direct personal care and assistance to adult day health care participants under the supervision of a Registered Nurse or Licensed Practical Nurse.
  • Assist participants with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, feeding, and mobility.
  • Observe, monitor, and report changes in participants’ physical, emotional, or cognitive condition to nursing staff.
  • Take and record vital signs and perform routine health observations as directed and permitted by scope of practice.
  • Support participants’ safe participation in therapeutic, social, and recreational activities.
  • Assist with positioning, transfers, ambulation, and use of mobility aids using proper body mechanics.
  • Maintain a clean, safe, and organized environment in accordance with infection control and safety standards.
  • Follow individualized care plans, physician orders, and program policies.
  • Document care activities accurately and timely in compliance with ADHC and Medicaid requirements.
  • Communicate effectively with participants, families, and interdisciplinary staff.
  • Follow emergency preparedness, safety, and participant rights procedures.
  • Promote participant dignity, privacy, and independence at all times.
