MAB Community Services is a non-profit human service organization serving adults with developmental disabilities, and adults with acquired brain injuries. We have an exciting opportunity for a detail-oriented, candidate who enjoys assisting individuals to reach their full potential. The Case Manager role will provide a rewarding opportunity to work with individuals in skilled nursing facilities to assist them with a transition to the community and ongoing community supports. The Case Manager role requires proven administrative skills, ability to submit documentation in a timely and efficient manner, proficiency with case management tasks. The Case Manager will work with individuals in the Worcester area and surrounding communities who are transitioning from skilled nursing facilities to living with supports in the community. The Case Manager should be prepared to transport individuals in their own vehicle on occasion and will require some travel to nursing facilities and in the community. This is a hybrid position with occasional travel to nursing facilities, client homes, community locations, and MAB offices.
