The Home and Community-Based Services (HCBS) Clinician provides clinical support, oversight, assessment, consultation, and evaluation for children’s HCBS programming. This role partners with team leaders to offer clinical guidance as needed, which may include: Conducting re-assessments for children open to any HCBS program every two years or sooner when clinically indicated. Developing and facilitating clinical groups to address the identified needs of the community. Providing clinical oversight, assessment, treatment planning, and discharge recommendations for the Daybreak Day Stabilization Program. Supporting HCBS interns to promote educational and professional development opportunities. Contributing to the development of individualized treatment plans, identifying evidence-based interventions, and assisting staff in implementing these practices with clients. This role ensures that evidence-based clinical practices are applied consistently, supports staff in delivering high-quality care, and promotes optimal outcomes for children and families served through HCBS programming.
