As a Clinical Transition Specialist, you will assist the Regional Directors of Clinical Services by providing hands-on clinical support to our communities which include Assisted Living and Memory Care. This support will include interim clinical leadership roles guidance during transitions clinical compliance quality reviews and targeted interventions that improve resident outcomes and readiness for surveys. Requirements

  • Associate degree or bachelor’s degree as a Registered Nurse
  • Minimum of three (3) years of Assisted Living Director of Clinical Services or similar nursing leadership experience.
  • Knowledge of and ability to conform to applicable laws, rules, and regulation.
  • Strong assessment skills clinical critical thinking and ability to support high acuity transitions.
  • Ability to foster a cooperative work environment and support conflict resolution among clinical teams.
  • Strong interpersonal and communication skills with the ability to work effectively with a wide range of constituencies in a diverse community.
  • Ability to work flexible schedules in a 24-hour operation.
  • Travel up to 90% of time with ability to support long term on-site assignment.
  • Ability to hold multistate Nurse License Compact required (NC, SC, VA, GA, TN, WV, & KY a plus)
