About The Position

The primary function of this position is to provide therapeutic services to consumers diagnosed with Behavioral Health Disorders. Responsibilities include, but are not exclusive to: individual therapy, psychosocial assessments, diagnoses, group therapy, treatment planning, psychoeducation, referrals and consultations.

Requirements

  • Provide effective individual therapy services to consumers receiving treatment for behavioral health diagnosis and/or substance use disorders
  • Initiate and maintain an individualized treatment plan, document progress in treatment, and ability to secure re-authorizations for services
  • Ability to assist and support consumers in developing/implementing a recovery plan
  • Comfortable conducting Tele-health and in-person comprehensive assessments, evaluations and developing treatment plans
  • Demonstrate clear verbal and written communication skills to peers, service providers, agencies and culturally diverse consumers.
  • Must be able to travel to all MyCare locations as needed
  • Must have excellent organizational and time management skills
  • Must maintain strict confidentiality in performing the assigned duties
  • Must be conscientious and respectful of others
  • Must be flexible and able to work autonomously
  • Possess cultural awareness and sensitivity
  • Must demonstrate a commitment to the mission of MyCare Health Center
  • Master's degree in Social Work required
  • Michigan state license required (LPC or LMSW )
  • 2+ years of therapy/counseling experience required; 5+ years of post-licensure psychotherapy experience preferred
  • Competency in assessment skills and evidence-based practices required
  • Computer literacy is required (EMR, Microsoft Office) required

Nice To Haves

  • Post-graduate continuing/advanced training preferred
  • CADC/CAADC/Development Plan preferred

Responsibilities

  • Provide individualized behavioral health treatment to consumers: including initial psychosocial assessment, diagnoses, individualized treatment plans and on-going therapy sessions
  • Maintain a therapy caseload that appropriately meets the needs of patients, utilizing evidence-based practices, best care practices, industry standards, organizational guidelines, appropriate accreditation standards, and applicable federal laws, including HIPAA
  • Refer consumers for more specialized or inpatient interventions as clinically appropriate.
  • Maintain quality care through participation in peer review, weekly case review meetings, policy development and quality improvement programs as requested
  • Complete all documentation in an appropriate, timely manner and in accordance with third party compliance standards
  • Maintain and update professional knowledge and proficiency through continuing education, staff meetings and other resources specific to providing integrated behavioral and mental services in a federally qualified health center
  • Responds to crisis situations according to policy
  • Schedule flexibility (e.g. one evening shift per week, ability to work from different sites as needed)
  • Participate in an integrated team approach to patient care/treatment. Treatment team will include physicians, RNs, peer recovery coaches, clinicians, etc.
  • Performs other task and responsibilities as assigned
