About The Position

This position provides support for all Service Coordination Department functions, including utilization management, service coordination, and population health management. It also involves providing health education and care coordination services for CCHP members, which can be delivered telephonically, in home, or in health facility settings. Cook Children's Health Plan offers vital coverage to nearly 120,000 people in low-income families who qualify for government-sponsored programs in a six-county service region, including CHIP, CHIP Perinatal, STAR (Medicaid), and STAR Kids Members in the Tarrant county service area (Tarrant, Johnson, Denton, Parker, Hood and Wise counties). Requirements

  • High School graduate minimum
  • Three years' experience in a managed care or medical office/hospital environment
  • Bi-lingual (English/Spanish) Required
  • Expert proficiency in conversational, reading, writing, and translating medical terms as well as diagnoses
  • Must pass Spanish test prior to hire
  • Superior verbal and written communication
  • Strong customer service skills
  • Strong phone skills
  • Familiarity with basic medical terminology and ICD-9 and CPT coding
  • Personal computer skills
  • Expertise with various software applications/databases (i.e., Microsoft Office)
  • Highly organized, ability to prioritize, and demonstrate moderately complex problem solving skills
  • Ability to work autonomously with minimal supervision in various settings
  • Basic service coordination concepts
  • Certified Community Health Worker

  • Associate degree preferred
