This position provides support for all Service Coordination Department functions, including utilization management, service coordination, and population health management. It also involves providing health education and care coordination services for CCHP members, which can be delivered telephonically, in home, or in health facility settings. Cook Children's Health Plan offers vital coverage to nearly 120,000 people in low-income families who qualify for government-sponsored programs in a six-county service region, including CHIP, CHIP Perinatal, STAR (Medicaid), and STAR Kids Members in the Tarrant county service area (Tarrant, Johnson, Denton, Parker, Hood and Wise counties).
