About The Position

The Clinical Supervisor provides leadership and acts as a clinical resource to the home health staff. The Clinical Supervisor reviews and maintains quality of patient care and documentation as it relates to standards, practices and regulatory requirement. The Home Hospice Supervisor is a licensed professional nurse who is a member of an interdisciplinary team serving the terminally ill patients in the hospice home care program. Utilizes established criteria for triage, responds to hospital requests for patient evaluation, performs admission and transfers to the IPU. Collaborates with physicians, Medical Director, NPs, and vendors to obtain necessary orders, medications, equipment and supplies for the patients' care. Provides supervision of the Hospice RN case managers and the Certified nursing assistants on the team. Assesses the competencies and skills of the team members, supports their orientation and training during onboarding and thereafter. Performs co-visits to evaluate performance. Mentors new employees, ensures their compliance with policies and procedures and adherence to the Plan of Care developed by the team. Makes assignments, provides coverage for absences. Communicates with case managers, Social Workers, and Chaplains to report issues for continuity of care. The framework for practice is steered by the WellStar Professional Practice Model, 5-Star Nursing and evidence based practice and research. RN competencies are derived from these models and supported by the WellStar Core Competencies. It is expected that all RN Clinical Nurses are licensed, knowledgeable and uphold the practice of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope Practice and Code of Ethics Standards put forth by the American Nurses Association. As a member of the patient services team, it is expected that the individual upholds the voice of the patient, department and system policies and procedures while supporting service excellence goals.

Requirements

  • Associates Nursing or Diploma (Nurse) Nursing
  • Bachelors Nursing-Preferred
  • BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
  • RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
  • GADL - Georgia Driver's License or VSDL - Valid State Driver's License
  • PVI - Private Vehicle insurance
  • Minimum 2 years acute care experience Required
  • Less than 1 year home hospice experience Required
  • leadership experience Preferred
  • Able to multitask in stressful situations able to work autonomously emotional maturity strong interpersonal skills effective written and oral communication skills excellent clinical assessment skills possess knowledge of developmental characteristics of people of all ages and the holistic approach to care possess knowledge of a wide range of disease processes competent in documentation in EMR.

Responsibilities

  • Supervises clinical field staff, including RNs, LPNs, Home Health Aides a. Monitors quality of care provided to patients and families and identifies opportunities for staff development. b. Monitors staff productivity. c. Coordinates staff availability for patient/family needs, including approval of PTO Requests. d. Oversees orientation of new staff, and annual competencies of existing staff. e. Mentors and supports staff with difficult cases f. Participates in joint visits with staff; conducts annual performance review.
  • Collaborates with interdisciplinary team. a. Participates in IDT to assure delivery of care by team members meets the patient and family goals and needs. b. Monitors for integration of clinical care with psychosocial and spiritual care. c. Assures efficient referral/admission process.
  • Assures implementation of regulatory guidelines related to clinical care. a. Assures that all guidelines are implemented in a timely manner. b. Oversees development and implementation of interdisciplinary plan of care. c. Collaborates with Home Hospice Manager to ensure quality of care delivered and that visit frequencies are met. d. Monitors documentation and coding to ensure that the patients process of decline is recorded
  • Participates in education about Hospice services and benefits. a. Consults with physicians and referral sources. b. Promotes Hospice through development of relationships with referral sources. c. Participates in System-wide committees as requested. d. Educates patients and families regarding hospice care and services leading to an informed decision for admission to hospice to include admission criteria, philosophy of palliative care, and financial aspects of hospice care.
  • Participates in performance improvement program. a. Identifies opportunities for improving clinical care. b. Identifies, initiates, and implements performance improvement plans. c. Oversees implementation of clinical aspects of department Performance improvement plan.
  • Participates on the Leadership Team. a. Supports mission and goals of Hospice. b. Participates in implementation of department goals. c. Provides leadership in planning and direction of Hospice. d. Supports the work of all hospice departments and teams
  • Assures financial accountability. a. Provides input into annual budget related to clinical care of home hospice patients. b. Manages staff resources, controls overtime and plans for maximum efficiency. c. Monitors expenses of supplies, medication, DME, and outside services (ER, visits, transports etc..) d. Suggests opportunities for improved financial performance
  • Performs other duties as assigned
