About The Position

Reviews applications and clinical reports with Clinical Manager and Program Director to determine program eligibility and suitability of applicant. Initiates and facilitates admission process. Performs service coordination for participant. Assists with conducting patient interviews, observing and obtaining patient's and record information. Creates clinical reports and coordinates various related administrative activities. Ensures patients are able to implement treatment plans and assists with any barriers preventing the ability to carry out such plans. Assists clients in obtaining needed psycho/social services. Employs problem-solving techniques, identifies crisis intervention matters, and uses proper case management and referral procedures. Requirements

  • Requires Master's Degree from an accredited school of Social Work.
  • Requires a Massachusetts Licensed Certified Social Worker
  • At least one year of relevant and recent experience working with disabled adults in a professional capacity is required.
  • Requires level of knowledge of Social Work process and community resources and agencies in relation to health, illness, and disability, generally acquired through one to two years on-the-job training.
  • Knowledge of community resources and agencies that serve the children and families.
  • Advanced interpersonal skills necessary to interact effectively with various healthcare professionals, to develop and maintain effective therapeutic relationships with patients and families, and to supervise graduate students.
  • Must be comfortable in a fast paced and dynamic work environment (sense of humor is valued).
  • Ability to work in an interdisciplinary team.
  • Cultural sensitivity and comfort with a wide range of racial and ethnic populations. Bicultural/Bilingual competency a plus.
  • Computer literacy with ability to use standard word processing, spreadsheet programs and e-mail (i.e. MS Word, Excel, Outlook) and web browsers.
  • Requires knowledge of Social Work process and community resources and agencies relative to health, protective services, crisis intervention, disability, etc.
  • Advanced interpersonal skills necessary to interact effectively with various healthcare professionals, to develop and maintain effective therapeutic relationships with patients and families, and to supervise graduate students.

  • Excellent English language skills to summarize and communicate orally and in writing moderately complex information in varied formats to internal and external customers.

  • Computer literacy with ability to use standard word processing, spreadsheet programs and e-mail (i.e. MS Word, Excel, Outlook) and web browsers.
  • Strong organizational skills, including ability to keep written documentation and to manage time effectively.
  • Effective English communication skills (oral and written). Additional language skills appropriate to the patient population served is a plus.
  • Computer proficiency to keep computerized records and prepare reports.
  • Excellent interpersonal skills to be socially perceptive in accepting and relaying confidential information, and cultural sensitivity and comfort with a diverse population.
  • Ability to problem solve issues when resources are stretched or scarce; ability to make alternative plans as needed.

Responsibilities

  • Takes all referrals for prospective participants. Promptly responds to all referrals and proceeds with admitting process as appropriate.
  • Initiates enrollment and discharge policies of the Program.
  • Performs all responsibilities of the Enrollment Policy in conjunction with the Assigned Nurse of the participant.
  • Performs follow-up on all discharged participants as required by Executive Office of Elder Affairs.
  • Completes Social history, care plan and discharge plan within 3 days of admission.
  • Arranges pre-admission interview at Adult Day Health Program and/or schedules home visits.
  • Consults with Program Director and Clinical Manager RN to review appropriateness of referral.
  • Disseminates preadmission information to all disciplines when application is complete.
  • Verifies eligibility of MassHealth insurance coverage for participants monthly.
  • Notifies and coordinates with homemaker agency, case manager and other involved team members of starting and participating days.
  • Conducts family conferences as needed to prevent foreseeable problems or deal with existing ones.
  • Progress notes on participants with any specific intervention, i.e., referral, consult, counseling.
  • Follows established hospital infection control and safety procedures.
  • Providing, as appropriate, coordination of the medically-related physical, mental, and behavioral, service needs of a participant at the time of enrollment, during treatment and care in the Program, or at the time of discharge;
  • Assisting with any personal, social, family, or adjustment problems the participant may experience at the Program;
  • Coordinating services if a participant needs services from other community agencies, and no other agency is acting as coordinator of services for that participant.
  • Performs EPIC scheduling and charge entry.
