The Clinical Social Worker III (LCSW/LMFT) provides psychosocial assessment, crisis intervention, counseling, brief therapeutic interventions, consultation, education, community linkages, supportive services, and facilitates the plan of care for patients with particular focus on high-risk populations. The CSW III acts as a consultant to the clinical team, service lines, community partners, participates in program development and quality improvement initiatives. Under the supervision of the Deputy Director, the Clinical Social Worker III will provide division-wide expertise, consultation, and oversight related to Housing Navigation. This role serves as a subject matter expert and resource to all programs, ensuring consistent, accurate, and effective housing navigation and eligibility practices across the division. They will provide training and in-services to all Citywide teams on housing and benefits navigation and updates. The incumbent will train and support staff in the use of One System, including access, documentation, and completion of housing assessments, and will ensure that housing-related data is accurately tracked and maintained. The incumbent will maintain a registry of client housing status, which will be used to proactively engage clinicians whose clients are unhoused or unstably housed. The Clinical Social Worker III will provide ongoing consultation to staff regarding housing navigation questions, barriers, and complex cases, and will support housing navigation training efforts division-wide. This position will serve as the primary liaison with the Department of Homelessness and Supportive Housing (HSH), ensuring the division receives timely and up-to-date information related to housing navigation policies, resources, and eligibility requirements, and that this information is effectively communicated and implemented across programs. The incumbent will also be responsible for maintaining and analyzing housing-related data to support program monitoring and improvement. In addition, the Clinical Social Worker III will provide expertise and support related to client eligibility and insurance. This includes developing and refining workflows to ensure clients maintain active health insurance and benefits (SSI/SSDI, GA, etc.), supporting staff with insurance and benefits navigation issues, and serving as a consultant and liaison for eligibility-related concerns. The incumbent will develop and maintain a system for monthly insurance verification and will track and manage data related to client eligibility to ensure compliance and continuity of care. The Clinical Social Worker III will also serve as an Epic Champion for the division, supporting best practices in clinical documentation, workflows, and system use. This includes providing guidance, training, and consultation to staff to promote timely, accurate, and compliant documentation. The CSW III has advanced clinical knowledge and skills and is distinguished from the CSW II by leadership activities including participation in advanced program planning, provision of educational programs, clinical supervision, consultation to the clinical team, other departments, and the community. The CSW III will support the development of new workflows and advancing the decision tree of options for vulnerable patients. CSW III will take lead along with Clinical Supervisor to train and onboard new staff. Additional CSW III duties include but not limited to monitoring data, participation in DPH State Performance Improvement Plans, keep stock of client resources, spearhead special projects and lead community case conferences. The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement. The salary range for this position is $45.19 - $78.06 (Step Hourly Rate).
