Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC.External candidates eligible for a $5000.00 Commitment Bonus paid over 1 year JOB SUMMARY Provides psychosocial assessments, intervention, and /or treatment related to the patient’s illness, and/or life situation. Identifies community resources to assist and meet the continuing needs of the patient and family in the home environment. May evaluate patients referred to the hospice program for admission

Requirements

  • Work requires a minimum of a Bachelors in Social Work from CSWE accredited school of social work.
  • 2years of social work experience, in a health care setting. Computer skills required with Microsoft Word and Excel experience preferred.
  • Valid NC   Driver’s License.
  • Working knowledge of Medicare and Medicaid regulations, including JCAHO and DFS standards, ICD-9 Coding.
  • Excellent written and verbal communication skills.
  • Strong interpersonal skills; ability to collaborate well.
  • Solid knowledge of social work practice: strong assessment, interview and counseling skills.
  • Strong knowledge of governmental and private organizations and community resources.

Nice To Haves

  • Masters of social work from an accredited school of social work, preferred.

Responsibilities

  • Maintains up to date knowledge of Medicare/Medicaid/JCAHO regulations.
  • Provide psychosocial assessments of patients and family to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.
  • Formulate, develop, and implement a comprehensive psychosocial treatment plan utilizing appropriate social work treatment and interventions.  Interventions may include crisis intervention brief and long-term individual, family therapy as well as grief and bereavement work.
  • Collaborate with the health care team and involve the patient/family in the development and implementation of plans.
  • Provide education and counseling to patients and families around issues related to adaptation to the patient’s diagnosis, illness, treatment and adaptation to end of life issues.
  • Maintain working knowledge of and liaison with community agencies; participate in area activities.
  • Assist with screening, identification and management of victims of abuse, neglect, domestic violence, rape, etc.  Make referrals as appropriate and serves as resource for the health care team
  • Will participate in on-call or after hour’s coverage.
  • Document assessment, plan, interactions and interventions according to departmental guidelines and standards.
  • Participate and represent social work perspective in multidisciplinary teams, rounds, etc.
  • Maintain a working knowledge of relevant medical/legal issues that impact on patient care, e.g., advance directives, child and elder abuse, etc.
  • Serve as patient advocate.
  • Maintain records and statistics in accordance with department policies.
  • Attend and participate in staff, committee, department, and other administrative meetings.
  • Keep current with social work and health care developments and seek to increase further enhancement of job related knowledge.
  • Provide education to other health care professionals.
  • Maintains confidentiality of employee and patient information.
  • Exhibits an attitude which promotes harmony and goodwill in the workplace.
  • Displays a clean, neat, professional appearance.
  • Participates in Performance Improvement Program.
  • Promotes quality, comprehensive services through a team approach.
  • Identities personal learning needs and implements corrective action plans to address those needs.
  • Conducts staff development programs to improve agency effectiveness.
