The Clinical Social Worker (MSW) provides direct patient and family support while coordinating psychosocial services across the cardiology, vascular surgery, and cardiothoracic surgery service lines. This role supports individuals with advanced heart failure therapies—including ventricular assist devices (VADs), chronic inotropic therapy, and heart transplantation—as well as patients undergoing complex surgical interventions such as lung transplantation and other major cardiothoracic or vascular procedures. The Clinical Social Worker conducts comprehensive psychosocial assessments and develops treatment plans aligned with the interdisciplinary care plan. Responsibilities include providing evidence‑based psychosocial interventions; offering education to patients, families, staff, and social work students; and addressing the unique emotional, logistical, and social challenges associated with advanced cardiac and thoracic care. This work encompasses the continuum of care for patients receiving Heart/Lung/VAD implantation, chronic inotrope therapy, vascular surgical treatment, and cardiothoracic surgical care, ensuring integrated and compassionate support throughout diagnosis, treatment, and recovery. The purpose of the Clinical Social Worker (MSW) position is to provide both direct patient care to patients and families and assist in the coordination of team delivered services. The Clinical Social Worker will complete a psychosocial assessment; develop treatment plans that are congruent with the overall health care plan; provide psychosocial intervention; educate patients, their family members, staff and social work students; facilitate compliance with treatment; work with the community to provide needed services; and coordinate discharge planning activities.
