About The Position

We serve all people, regardless of income or insurance status. Affinity Health Center seeks a vibrant and motivated Clinical Quality Coordinator to champion our patient care quality program. The coordinator will use strong relationship building skills to collaborate with primary care clinical teams and leaders on strategies to support closure of care gaps and positive patient health outcomes as measured by standardized indicators. The highly organized Coordinator has a detailed understanding of health care quality goals in closing care gaps, monitors performance and tracks agency data performance in comparison with regional and national standards. The Coordinator uses Continuous Quality Improvement methodology to develop and implement processes and facilitate initiatives across the organization to ensure that Affinity Health Center has a pathway to help close patient care gaps. Lead Strategic Quality Initiatives : Champion performance on quality metrics while designing and implementing improvement strategies that elevate patient outcomes. You'll monitor what matters and mobilize teams to achieve excellence in both primary and HIV care delivery. Empower Through Technology Training: Collaborate with our data and quality teams to ensure staff mastery of our EHR and population health management systems. You'll turn technology from a burden into a powerful tool for better care. Harness Data for Impact: Analyze clinical data to identify opportunities and drive process improvements that directly enhance patient outcomes. Your insights will shape how we deliver care. Eliminate Barriers, Elevate Safety: Lead quality assurance efforts that reduce care errors, remove delays, and dismantle obstacles to accessing care. You'll help steer our organization toward value-based care models that prioritize what patients truly need. Amplify the Patient Voice : Oversee patient experience surveys and quality reviews, translating feedback into meaningful improvements that enrich every patient interaction. Restore Joy in Practice : Optimize clinical workflows and EHR utilization to eliminate redundancy, reduce administrative burden, and ensure every team member works at the top of their license. You'll help reclaim time, restore purpose, and make our organization the place where healthcare professionals want to practice—and patients want to receive care.

Requirements

  • High school diploma or general education degree (GED) required
  • Reliable transportation required

Nice To Haves

  • Experience in healthcare/clinical quality management
  • Electronic health record experience
  • Relationship building experience partnering with primary care clinical teams and leaders on strategies to support closure of care gaps and positive patient health outcomes.
  • Detailed oriented with experience understanding of health care quality goals in closing care gaps, monitoring performance, and tracking agency data performance in comparison with regional and national standards.
  • Experience using Continuous Quality Improvement methodologies to develop and implement processes, along with facilitating initiatives across the organization, to ensure Affinity Health Center has a pathway to help close patient care gaps.
