About The Position

The LPN Clinical Coordinator is crucial to the day-to-day patient flow as part of a healthcare team by assisting in the planning and implementing care interventions while promoting high quality patient care in a professional, efficient and safe manner. General Job Functions: Assists in the planning and implementation of interventions to ensure evidence-based practices are being adhered to. Directs the team member as needed to support and maintain high-quality patient care. Works closely with providers, supervisors, support staff and other members of the organization to ensure seamless workflow, safe patient care, while supporting the operational needs of the business. Must be able to effectively communicate with all team members to support a cohesive work environment. Works collaboratively with the department manager to ensure compliance with Summit Health policies, and regulatory standards and guidelines (OSHA, DOH, HIPAA, etc. Assists with the training and orientation of new staff. Expected to keep relevant knowledge up to date and apply it to everyday practice, in addition to assisting with other duties as assigned within their scope of practice.

Requirements

  • Current BLS certification
  • Minimum of 3 years LPN clinical experience
  • Current/active New York State licensure as a Licensed Practical Nurse
  • Strong computer skills
  • Strong organizational skills, able to multitask in fast paced environment
  • Excellent communication skills: both written and verbal
  • Excellent time management skills
  • Assist in maintaining inventory, ordering supplies, and ensuring equipment functionality
  • Able to identify knowledge deficits of staff
  • Ability to prioritize, organize and manage multiple activities with limited supervision
  • Ability to work both independently and as part of a team
  • Support and mentor clinical staff members in providing safe and effective patient care.
  • Ability to think critically, motivate staff to meet department quality goals, and work with minimal supervision in a stressful environment
  • Promote a culture of accountability, professionalism, and respect across the care team.
  • Experience with EHR required
  • One year leadership experience
