About The Position

Clinical Program Coordinator II/ CMA Job Summary: The Clinical Program Coordinator II/ CMA is a key team member of the Johns Hopkins Home-based Medicine (JHOME) program. The JHOME program is a primary care program that provides home medical visits for older patients who can no longer reach a medical office. The CPC II/CMA will have a dual role of being the program coordinator and also the medical assistant for the program. As a CPC II, their duties will include direct patient-facing tasks, as well as office management and administrative support. The candidate must have experience interacting directly with patients, as well as organizational and data management skills. As a CPC II, they will be responsible for the acceptance and scheduling for all JHOME patients, and ensuring coordination of care between all disciplines. The CPC II will be responsible for scheduling patient visits; obtaining prior authorizations; screening new patients using clinical criteria; creating EPIC referrals for new patients; maintaining a patient database; tracking demographic and clinical data to produce weekly and monthly reports; managing insurance/registration information; managing JHOME educational program scheduling for fellows, residents, medical students, and other learners; providing coverage for the JHOME phone line and carrying out other related duties. Because all JHOME appointments are home visits, scheduling also includes mapping and clustering the patients. The CPC II will also be responsible in maintaining an inventory of clinical supplies. As a CMA, they will be responsible for drawing blood; obtaining and recording patient's vital signs: temperature, pulse, respiration, blood pressure, basic wound care, immunizations, performing and reporting basic physical assessments such as leg edema and breath sounds. Relays messages to providers and provides appropriate patient education as directed. CPC II/CMA is a valued member of the JHOME team and is expected to fully participate in both administrative and patient care team meetings.

Requirements

  • 3 years experience in health care related field, home health or geriatric care experience preferred
  • Certification/Registration as a Medical Assistant (e.g. CMA, AAMA, RMA, NRCMA, NCMA, RCMAS, CCMA, AMT, and NAHP) and CPR required
  • If not currently certified/registered, must become certified/registered within 90 days of date of hire.
