Requirements

  • Graduate of an accredited college/university of pharmacy (PharmD or B.S. with equivalent experience)
  • Completion of ASHP accredited PGY-1 Pharmacy Residency.
  • Valid license from the Board of Pharmacy of the State of Texas.
  • Certification in Basic Life Support / First Aid / CPR.
  • 2+ years of medication adherence counseling experience.
  • Computer experience with Windows, Microsoft Word & Excel.
  • Must be able to manage multiple deadlines and prioritize.
  • Excellent communication skills and customer service skills.

Nice To Haves

  • Experience in HIV/AIDs, Geriatrics, and/or Pediatrics is preferred.
  • Completion of ASHP accredited PGY-2 Pharmacy Residency in Ambulatory Care, Adult Medicine, Geriatric Medicine, or Pediatrics is preferred.
  • Certification in immunization delivery or smoking cessation is preferred.

Responsibilities

  • Provide pharmaceutical care services through direct patient and provider interactions.
  • Conduct chronic disease state medication management services via face-to-face or virtual care.
  • Obtain comprehensive medical and medication histories to ensure safe and effective care.
  • Reconcile patient medications during ambulatory care clinic visits.
  • Perform physical assessments and triage patients as needed.
  • Document encounters and interventions in the electronic medical record.
  • Design, implement, and monitor therapeutic plans under your scope of practice.
  • Initiate, modify, renew, refill, or discontinue medication therapy.
  • Order labs and tests to determine the efficacy and toxicity of medication therapy.
  • Document and report adverse drug events and medication allergies.
  • Provide drug information and education to patients, families, and providers.
  • Assess medication adherence during counseling sessions.
  • Set priorities and work independently while collaborating with others.
  • Serve as a preceptor to pharmacy students and residents.
  • Perform other duties as assigned.
