Requirements

Graduate of an accredited college/university of pharmacy (PharmD or B.S. with equivalent experience)
Completion of ASHP accredited PGY-1 Pharmacy Residency.
Valid license from the Board of Pharmacy of the State of Texas.
Certification in Basic Life Support / First Aid / CPR.
2+ years of medication adherence counseling experience.
Computer experience with Windows, Microsoft Word & Excel.
Must be able to manage multiple deadlines and prioritize.
Excellent communication skills and customer service skills.

Nice To Haves

Experience in HIV/AIDs, Geriatrics, and/or Pediatrics is preferred.
Completion of ASHP accredited PGY-2 Pharmacy Residency in Ambulatory Care, Adult Medicine, Geriatric Medicine, or Pediatrics is preferred.
Certification in immunization delivery or smoking cessation is preferred.

Responsibilities

Provide pharmaceutical care services through direct patient and provider interactions.
Conduct chronic disease state medication management services via face-to-face or virtual care.
Obtain comprehensive medical and medication histories to ensure safe and effective care.
Reconcile patient medications during ambulatory care clinic visits.
Perform physical assessments and triage patients as needed.
Document encounters and interventions in the electronic medical record.
Design, implement, and monitor therapeutic plans under your scope of practice.
Initiate, modify, renew, refill, or discontinue medication therapy.
Order labs and tests to determine the efficacy and toxicity of medication therapy.
Document and report adverse drug events and medication allergies.
Provide drug information and education to patients, families, and providers.
Assess medication adherence during counseling sessions.
Set priorities and work independently while collaborating with others.
Serve as a preceptor to pharmacy students and residents.
Perform other duties as assigned.
