This is achieved by serving as the informatics team member responsible for the clinical and operational maintenance of technology to enhance use of medication information and safety. Under the direction of the Pharmacy Leadership Team, the Clinical Pharmacy Specialist Informatics assists in coordination and validation of clinical data in various systems, implementation of new systems, and maintenance of current systems.Job Description:Essential Duties & Responsibilities: including but not limited to:

Requirements

Education: Must be a graduate of an accredited college of pharmacy with a current MA registration in Pharmacy
Licensure, Certification, Registration: Must be eligible for a Massachusetts Pharmacist license, advanced certification (e.g. BPS) in applicable area within 2 years of hire.
Skills, Knowledge & Abilities: Requires a working knowledge of all basic aspects of health-system pharmacy (e.g. drug therapeutics, sterile compounding, etc.)
Experience: PGY 1 + 1-3 years or PGY 1 + PYG 2 or equivalent

Responsibilities

Clinically reviews and assesses all medication orders to ensure that medications entered into patient profiles are safe for administration. Assessment should include appropriate drug selection, medication allergy, and dose, route of administration, diagnosis, relevant laboratory results, therapeutic duplication, drug interactions and age of patient.
Performs assigned work during shift in an efficient and timely manner.
Practices in accordance with the state and federal laws, rules, and regulations, including but not limited to Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and Drug Administration, and the Drug Enforcement Administration. Maintains certification and credentials in accordance with the Board of Registration and adheres to institutional policies and procedures.
Reviews and verifies medication orders with consideration, including but not limited to the following patient specific parameters: diagnosis, weight/body surface area, age, allergies, organ function, laboratory values, medication duplication, drug interactions. Clarifies orders deemed to be inappropriate, incomplete, or otherwise unclear with the prescriber in a timely manner.
Documents and communicates pertinent information and discussion with the healthcare team and pharmacy staff to ensure continuity of care.
Provides drug information to physicians, nurses and other health care professionals.
Checks and verifies sterile and non-sterile medications.
Supervises the activities of students, technicians, and residents within the pharmacist area of practice.
Promotes a culture of patient safety by submitting medication related events and near misses.
Completes daily pharmacy surveillance tool monitoring, including follow-up and documentation, as well as other daily patient care initiatives and relevant reports.
Works collaboratively with all pharmacists, pharmacy technicians, and pharmacy personnel. Upholds a culture of respect and teamwork across the Pharmacy Department.
Precepts pharmacy students, resident rotations, and resident-led quality improvement projects. May serve as Residency Program Coordinator or Residency Program Director. Serves as a role model and training mentor for other staff.
Demonstrates competency for Clinical Pharmacy Specialist I and in all practice areas in which the Pharmacist's duties are performed. Has completed all required departmental core competencies for areas of practice and specialty as applicable.
Actively participates in continuous performance improvement activities of the department, including providing suggestions on how to improve day-to-day operations.
Regularly develops and leads departmental initiatives including but not limited to supporting training & education, quality, medication stewardship, presenting requests at Pharmacy & Therapeutics Committee, creating and maintaining policies and procedures, conducting assessments of workflows, and/or developing departmental tools and resources.
Supports a specific core area of practice through interdisciplinary education, ongoing specialty training and as a resource for Pharmacy staff.
Develops and leads updating clinical guidelines, policies, procedures and presents hospital wide educational initiatives.
Serves on and may lead hospital interdisciplinary committees or workgroups as needed to improve process and quality outcomes.
Leads IRB-approved research activities, medication use evaluations, or cost-savings initiatives.
Supports manager/director with administrative responsibilities such as but not limited to chairing departmental committees, scheduling, staff coordination, budgeting and may participate in the Pharmacy Administrator on Call rotation.
Incorporates Lahey Hospital and Medical Center Guiding Principles, Mission Statement and Goals into daily activities.
